Have Vaginismus. Tear At Vaginal Entrance. Cause?

I have a little tear at the entrance of my vagina, i have vaginsmus and i am using dilators to overcome my vaginismus.. i could insert the biggest dilator after a few weeks,but then i stopped dilating for 3weeks, and when i tried to put in the biggest dilator it made a tear in my vagina. Is vaginismus the cause or could it be that i have a small vaginal introitus.. when i look at my vagina at the mirror the vaginal entrance looks very small to me. also when i insert a dilator it seems to hurt at the entrance more than the rest
