Have Pain Radiating From Hips To Legs. Family History Of Soft Tissue Arthritis. Give Me Suggestion

I have pain radiating from my hips down both legs. Sometimes just in hip joints. Pain feels like it is in my bones. Keeps me awake at night. Also have some sharper muscle ? Pain which is sharper below knees and into feet?

Family history of soft tissue arthritis. Sister has had hip replacement at age 60. I am 54 and not over weight ...

Hi, thanks for writing to HCM.

Pain originating from the hip region and radiating down into the legs and into the feet is highly suggestive of Lumbar disc disease in which a protruded intervertebral disc will be pressing over the nerve root causing typical symptoms.

An MRI scan of the lumbar spine will help in establishing the proper diagnosis. An xray of the hip joint is also mandatory to rule out any arthritic changes, which also present with similar symptoms but the pain seldom radiates below the level of knee joint.

The conservative management of lumbar disc disease is as follows -

- Maintain optimum body weight,if you are obese
- Avoid lifting heavy weights as it imparts more strain on your back
- Avoid excessive forward bending activities as it will worsen the disc protrusion. if you want to bend forwards, do it with your hips and knees flexed.
- Use a lumbosacral back support to stabilize your spinal muscles
- Intermittent pelvic traction might be helpful to cause a regression of the protruded disc
- Physiotherapy is needed to strengthen your back muscles. Physiotherapy has to be done only after the acute pain has subsided.
- Use analgesics like advil or aleve and muscle relaxants as needed. Vitamin B12 supplements & Pregabalin may be helpful.

If your fail to improve with the above measures, an Epidural steroid injection may provide good symptom relief for a few months.
