Have Multiple Health Issues. Can I Consult A Doctor Online?

Patient’s name : Mrs. Chhabi Dutta Present status : Retired 70 yrs old school-teacher, widow last 3 yrs, stays alone P/C : 1. Dm – 2 2. DEP. 3. Gait Imbalance – Non-organic Medicines : 1. Tryptomer (25) 2. Daxid (100) 3. Asomex-TM 4. Naprosyn (250) 5. Volix (0.3) 6. Gemcal (500) 7. Methycobal (500) Present Complications : 1. On 1st week of October, ’13 suddenly feeling too much weak  pressure fell down, WBC count abnormally increased, antibiotic (prescribed by local MD) given for 5 days and pressure’s medicine (Asomex-TM) stopped temporarily; then test report shows normal WBC count; but Hb low (9) and Sugar (F) only high. Then we went to a family trip to Rajasthan in puja vacation, there as such no problem arose to her except one time falling down due to wrong-stepping with irritated mind. 2. Mental agony and “wrong-treatment causing her husband’s death” –– this self-view generates angriness and guilty-blaming-annoyance in her mind. She is writing diary about that and regularly goes through those pages; compare others’ death with her husband’s. 3. Her one tooth broken, but she refuses to consult doctor. 4. Regular constipation, loss of appetite; and then loose motion and gas-acidity. 5. No food is tasty to her, never wants to eat. 6. Many time feels breathlessness, loss of balance during walking. 7. More information is required for reaching to a definite conclusion:-

- Blood sugar levels : fasting, 2hours EACH AFTER breakfast, lunch and dinner and at 3AM (at night).
- Blood pressure reading as soon as she wakes up
- Blood Urea, Creatinine levels
- Blood Vit D levels
- Chest X-ray
- ECG, 2-D AND STRESS Echo
- MRI Brain

From the information provided, your mother is probably suffering from:-

(a) Diabetic complications :- autonomic neuropathy, ongoing Vitamin B12 deficiency; recurrent hypoglycemia, peripheral polyneuropathy.

(b) Chronic severe mental depression with possible early dementia

(c) Hypertensive heart disease or coronary artery disease, with impending heart failure (in view of her dry cough during sleep at night)

(d) Pulmonary tuberculosis.

(e) Osteoarthritis

Consult your doctor and apprise him of my opinion. I am certain that he will agree with me, order required tests, and- based on the reports- will prescribe suitable/modified treatment as advised.
