GFR Has Been Fluctuating. It Goes Down When There Is Diarrhea. His Ultrasound Showed Minimal Cortical Thinning. Treatment?

My husband's GFR has been fluctuating a lot in the last two years. It has ranged between 22 and 40 but mainly under 30. Every time he has diarreah or his blood pressure is uncontrolled his GFR goes down. Both these events happen often. Doctos are unable to find cause of diarreah and it seems a constant battle to get his BP medication right, especially recently when he was found to have Potassium levels of 7.8 and had to be taken off two of his medications. They are now trying to control his BP all over again. His ultrasound showed minimal cortical thinning (.9) and increased echogenicity. Yesterday Potassium back to normal and GFR up to 32 from 22 two weeks ago. I realise that there is a base line GFR for his chronic CKD which goes down every time there is some sort of acute damage, but what I need to know is, every time there is an acute episode, will his GFR ever get back to the previous GFR levels or will each episode gradually bring his baseline down each time. He seems to go from stage 4 to stage 3 and back again quite often. I wnat to know if he should be preparing himself for stage 5 in the near future. Thank you

Thanks for the query to H.C.M. Forum.
Since your emphasis is only over G F R .
G F R ( glomerular filtration rate) in males 90-124 mL/minute.
Since many factors are responsible for this filtration rate as minimal cortical lining in kidneys and increased echogenicity.
The important thing is level of serum creatinine and blood urea as these two parameters are very important for assessment of kidney function.
As far as concerned about recurrent diarrhea , colonoscopy may reveal something.
Idiopathic diarrhea may be there.
upload all reports as, urine, stool blood ultrasound then it will be possible to find out any disease.
