Hi there.
i was wondering what, if any, health implications there may be with oral sex with my fiance. She insists i go down on her, but also she likes me to go a little further round to her anus. Should we only carry out such oral activities after showering or is it fine at any time of the day?

yes after taking shower you can do oral sex.
but if any one of you against oral sex not perform with force.
There is no ill effect of oral sex on health.
