General & Family Physician 's  Response
Hi
Microangiopathic ischemic changes mean that small blood vessels are blocked. Yes it causes stroke or mini-strokes. Stroke does not cause chronic severe headache rather they are associated by weakness of a limb or one half of the body or half face or sometimes blindness & sometimes these symptoms can be transient & that relive within 24 hours. You better take him to neurophysician for proper workup. Strokes can be caused by long standing hypertension, diabetes, certain cardiac problems which cause clots formation in small vessels of the brain.
Hope my answer satisfies you.
Take care
