Hi welcome to HCM.
A strong family history of stroke and heart attack warrants further investigation in the form of baseline EKG and 2 dimensional echo to rule out cardiac ischemia. In all cases you to visit the ER at the earliest.Low blood pressure can cause dizziness.Drugs like sublingual nitrates and antiplatelets like clopidogrel and aspirin will help to reduce the chest pain.Consult your cardiologist for further care.
Thank you
