Does Blood In Stools Suggest Colitis?

I had colitis a several years ago. I passed quite a bit of blood in the stool.I am experiencing similar symptoms today. Stomach cramps, diareah which later went away and now only small amounts of blood when using the bathroom.Is there something I can do at home to help this if it is indeed another bout with colitis?

Diverticulitis may cause blood in stool.
Intestinal poly may cause blood in stool.

Hemorroid or anal fissure may cause blood in stool problem.
