Diagnosed With Anal Fissures, Sentinel Piles, On Menses, Taking Combiflam. Help?

Hello, I am 25/Female, with large sentinel piles and also diagnosed with anal fissures, the problem that I am facing is not with piles, rather than fissures. I had many sleepless nights, now since last two days Combiflam seems to work, and i am sleeping properly. I am have menses too, can this be reason for fissures pain? I want to avoid surgery and medicines, however I am forced to take Combiflam, I was also applying Anobliss, but seems to irritate the fissures as soon as I insert the applicator and I have stopped using that since last two days. Please help

Hello! Thanks for putting your query in HCM. I am a Gastroenterologist (DM).

You haven't mentioned where you are feeling pain. If this is present in abdomen especially lower part then it may be due to menses. And if the pain is in annum especially while defecation then it is due to anal fissure

For fissure:
1. Make your stool soft by taking a laxative like syrup cremaffin plus and lots of fibres by taking fruits and vegetables and may also add psyllium fibres like fibril
2. Local ointment containing diltizem for application per anum before and after defecation. And this is to be applied at the outer surface of the anal verge with finger
3. Sitz bath
