Diabetic, thyroid patient. Blood reports show raised TLC and ESR. Suggestions ?

HI DR ...MY MOTHERS AGE IS 58 YEARS..SHE IS DIABETIC PATIENT AND THYROID PATIENT......RECENTLY WE PERFORMED HER TESTS URINE: REACTION- ACIDIC SUGAR- NT DETECTED RBCs- Nil Pus Calls- Nil 2. Blood-LPT, Urea ,S Biluribin.Seroi,PT Lipid Profile: Total Cholestrol-234 Trigly Cerides-135 LDL Cholestrol-147 HDL Cholestrol-40 Total HDL Ratio- 5.8 SERUM ENZYMES: SGOT-19 SGPT-21 UREA- 26 Mg/dL Creatinine- 0.9Mg/dL Cholestrol- 234 LPT : BILURUBIN-N0.9 BURGH REACTION-NEGETIVE 3. Hb, TLC ,DLC,ESR BLOOD: ESR 32mm falls at one and of first hour(WINTROB S) Haemoglobin 13.2gm% TLC 0000/cmm DLC: Poly- 61% Lyngda-33% Mons-02% Eosino-04% 4. Sugar F,PP Fasting- 141 PR- 154 TLC 0000 TLC AND ESR RAISED.....WHAT U SUGGEST??

Dear Dr Nidhi,

ESR of 32 mm is just not a matter to worry. pay attention if her thyroid is normal, one because total and bad cholesterol- LDL are very high for a person with diabetes and if her TSH is higher that might be the reason for the same. And if the TSH is normal then she immediately needs to be put on antilipidemics without delay. Sugars- the fasting one prima facie looks slightly high but please get HbA1c done as that will certainly give idea if her overall control is fine or she needs titration and/ or changes in anti diabetes medications. Also because she has diabetes, get urine micro albumin and dilated fundoscopy to rule out other complications of diabetes. I hope this will guide you for the further course of action to be taken.
