DEAR SIR/MADAM

THERE ARE MANY REASONS FOR WOUND IN DIABETES. IF THE WOUND IS ASSOCIATED WITH ALTERED BLOOD FLOW TO THE LIMBS, IT IS ASSOCIATED WITH RISK OF DEVELOPING INTO GANGRENE.
PLEASE LOOK FOR THE SIGNS OF ISCHEMIA (POOR BLOOD FLOW) TO THE LIMBS LIKE COLOR CHANGES IN THE SKIN, ALTERED SENSATIONS AND MOBILITY.
IF THERE IS ALTERED BLOOD FLOW TO THE LIMB THEN USE OF TOPICAL CREAMS WILL BE OF NO USE PRACTICALLY.
I SUGGEST YOU TO CONTINUE YOUR ANTIBIOTIC DOSAGES. IF YOU WANT TO APPLY ANY TOPICAL CREAMS/PREPARATIONS, THEN YOU CAN USE FUSIDIC ACID OR NADIFLOXACIN.

Internal Medicine Specialist Dr. Mayank Bhargava's  Response
Jan 2013
Hi,
Thanks for posting your query.
With the available described symptoms, appears to be possibility of diabetes induced complication.
Diabetes is a multi organ disease with involvement of almost all organs of body due to its complications.
Blood sugar both fasting and postprandial must be in controlled range.
Diabetes is due to progressive loss or destruction of beta cells in endocrinal part of pancreas.
Uncontrolled Diabetes predisposes to accelerated atherosclerosis in peripheral arteries.
Decreased blood supply with associated diabetic neuropathy may predispose to diabetc foot ulcer formation.
You should consult with internal medicine specialist/ endocrinologist and should go for thorough check up.
She should also go for postprandial sugar and glycosylated hemoglobin, lipid profile.
Her blood sugar must be in controlled range.
She can get better relief after taking Cilastazol 100 mg BD and by applying hot fomentation.
Take care,
Dr. Mayank Bhargava
