welcome to healthcare magic,
the pink spots will reduce if you stop smoking, you can start reducing the no of cigarettes you smoke eventually leaving it, they are nicotine gums available which help to reduce the smoking habit.
for the back pain we need to investigate by doing an xray, if the xray shows something then we might need to do a ct scan to see if there is any compression of nerves.
you could take a painkiller for the backpain and massage with painkiller ointment.
physiotherapy will also help.
hope to have answered your query

Dentist Dr. Farah Hussain's  Response
Hi and welcome to HCM,
Smoking is indeed a very injurious habit . It has got destructive effects on the oral health as well as general well being of an individual.
Tobacco smoking causes many respiratory disorders and oral conditions like gingival inflammation, periodontistis , tongue and lip disorders and malignancies , leukoplakia and other cancerous lesions.
So considering these , please try and get rid of this habit.
Most important is your COMMITMENT towards quitting,
Your preparation and self motivation,
you can use habit breaking medicines available like nicorett tablets or chewing gums which help in quitting the habit
i hope this helps,
take care.
