Constipated, unable to urinate. Have hemorrhoids and anal fissure. Had THD. Help?

Had THD on internal hemorrhoids , 2 external hemorrhoids removed, and a anal fissure fixed on 2/8/13. Still bed ridden, I am now constipated(but do have anal leakage) and can not urinate. DR. Appointment in morning, but in pain. Took milk of magnesia with no help. When I try to dedicate it feels like I am ripping in half.
