my husband has a cough for over 3 weeks that gets worse at night or when he lies down. he does not have trouble breathing, is not congested, or have a fever or a runny nose. he has continued to work (physical labor) and he is a smoker.

Smoking can cause irreversible damage to lungs which lead to several lungs disease like: COPD, emphysema, infeaction and cancer etc.

Chronic cough may occur due to COPD and infection. It is necessary for him to stop smoking. If he is not able to quit smoking on his own then he take help of nearby psychiatrist. Same time get him treated for his respiratory problem.
