Topical retinoids are comedolytic; they increase skin turnover, therefore during the initial part of therapy with topical retinoids previously unseen lesions which were situated deep may come up to the surface and cause a flare. This is expected with topical retinoids, specially during the early part of therapy i.e initial 2-4 weeks. The flare usually subsides over the next 2-4 weeks with continued treatment. I would suggest that you continue with the topical gels as prescribed. You may seek another appointment with your dermatologist if the flare is bothersome. Your dermatologist might choose to prescribe oral antibiotics such as either Doxycycline Or Azithromycin, which would help to reduce inflammatory acne as well as take care of the flare. Regards
