Dear Mam

I require more history to draw a conclusion. Is he having any fever , weight loss , cough with expectoration , any swollen glands , any history of recurrent eye redning , joint pains.
Any alcohol consumption?
High ESR , can be a sign of inflammation.
Since how long is he having this? Any chest pain?
I would like to rule out , autoimmune conditions , Tuberculosis , lymphoma.
It might be simply because of some infection.

Need these details to help you further.
