Intermenstrual bleeding with painful intercourse can be due to cervical lesions particularly when there is post coital bleeding (after intercourse) It can occur in benign cervical lesions like cervicitis, polyps or erosions of cervix. But it is also a symptom of cancer of the cervix. Also you give history of severe pain with foul smelling discharge mixed with blood after intercourse which is seen in cancer cervix. Your problem has lasted for an year now which is quite significant. Dont worry as the earlier a diagnosis is made , the better it is. I want you to meet a gynecologist for an internal examination. Also get a Pap smear done if there is no visible growth on cervix else get a tissue biopsy. As you say your last ultrasound showed thickening of uterus lining which indicates you have endometrial hyperplasia which itself can cause irregular bleeding. To rule out cancer of the lining which presents similarly, you should get an endometrail biopsy with biospy of any visible growth of cervix. Take care
