Brief Answer:
I would explain as follows:

Detailed Answer:

Hello again, dear XXXXXXX

I would explain that performing a coronary angiography is one of the preliminary tests before proceeding to valvular replacement or repair.

This test is necessary to be performed in all the adult patients (>40 or 45 years old depending on the clinic protocol), especially in those with known or highly suspicions of having coronary artery disease.

In such way, would be possible an appropriate strategy of dealing with potential coronary and also valvular disorders.

If coronary disease is present, percutaneous coronary angioplasty with stent implantation may be tried first, if feasible.

But, if diffuse and complex coronary lesions are concluded after angiography, then CABG (coronary bypass) with concomitant valvular replacement would be the recommended procedure.

Hope to have been helpful to you!

In case of further uncertainties, do not hesitate to ask me!
