Brief Answer:
Test for Hiv at 4 weeks; groin rashes could be tinea

Detailed Answer:
Hello. Thank you for writing to us.

I have taken note of your query.

A broken condom during sex is not considered a safe sexual act as far as risk of exposure to Hiv is concerned, even though you pulled out soon.

I suggest you to take up a screening test for Hiv at 4 weeks, preferably a Hiv Combo test, which would test for both P 24 antigen as well as Hiv antibodies.
This test has a sensitivity of 99.8% at 4 weeks.

Regarding your second concern of groin rashes I would like to consider a possibility of fungal infection/tinea.
Kindly upload an image so that i am able to confirm and guide you further regarding that.

Brief Answer:
Rashes in groin are likely to be fungal

Detailed Answer:
Hi.

Most likely the rashes are fungal in origin..
I would suggest a topical cream containing an antifungal preferably in combination with a topical steroid e.g clotrimazole 1%+beclomethasone 0.025% cream, twice daily.

HIV status of sex worker is not reliable because there is a likely possibility that she may have acquired Hiv infection from someone after she was tested for Hiv & therefore you cannot be sure about that.

It would be better for you to test for Hiv and put all doubts to rest.

Brief Answer:
Testing at 4 weeks with HIV DUO test has almost 100% sensitivity

Detailed Answer:
Hi.

Most of individuals who are infected with HIV will have circulating P 24 antigen and will develop detectable antibodies to HIV at 4 weeks.
Therefore, at 4 weeks most of the infected individuals can be detected by HIV DUO test.
The HIV DUO test which I mentioned has a sensitivity of 99.8% at 4 weeks i.e 99.8% of HIV positive individuals at 4 weeks...means it would detect most of the infected individuals
The sensitivity increases to 99.9% at 12 weeks. Therefore, the recommendation is to repeat this test at 12 weeks/ 3 months. Negative test at 12 weeks in enough conclusive evidence that an individual is HIV negative.
Thereafter there is no need for any further test and no need to repeat at 6 months.
Testing at 6 months is only advisable in few special circumstances like when the patient is immunocompromised due to illness Or is on oral steroids e.g in autoimmune disease Or is oral immunosuppressives like organ transplant recipients.
These states would interfere and thus delay the development of antibodies to HIV. Only in these special circumstances is testing at 6 months indicated.
There is no need for any medication for you. Please don't be anxious.
