What Is The Treatment For Enlarged Adenoids Apart From Surgery?

Question: Sir, My 3.5 years old boy kid is having severe nasal congestion and takes breathing from mouth especially during night and sleeping hours. Nasal decongestant live otrivin etc. are not effective on him. His pediatrician has said that he has allergy and prescribed him Teleacast kid tab-one table a day for 3 months. He is getting this medicine for alomost one month and still continuing but his nasal congestion continues till date. Meanwhile on 1st january 2014, i consulted an ENT specialist,who after an X-ray,said that he has enlarged adenoids which is blocking his air ways through nose. He prescribed a nasal steroid Meta Spray -one spray in each nose twice daily and said that if this will not help surgery is the last option. But even after almost one month of its use, he has got only aroung 30% relief. Now my question is -what is the treatment for enlarged adenoids apart from surgery-2. Is there any side effect of using this nasal steroid for prolonged time like two,three months like suppression of immune system etc. in the prescribed dosages as has been prescribed by my ENT specialist. My kid also has problem of tonsils. Please suggest me comprehensive details and treatment of problem along with advice for about using nasal steroid.

Brief Answer: Surgery is a better option. Detailed Answer: Hi, Thanks for the query. Surgery is the treatment of choice for adenoids which are symptomatic. Intranasal steroid sprays are a very safe form of treatment and are an established form of treatment for allergic rhinitis. Sprays for treatment of adenoids is not a long term option if the obstructive symptoms are because of that and surgery is a better option. There are few side effects but suppression of immune system with present set of sprays is not there usually. Tonsil surgery in patients with allergy can be a two edged sword and the decision will depend upon many factors and pattern of symptoms over a span of time. So, surgery for adenoids is a very effective and better way of treating the problem. Feel free to ask another query. Regards

sir, but patient ,my kid is showing some sign of improvement and i want to clarify if i can continue nasal steroid for more than one month without having any major side effects.

Brief Answer: You can safely use it Detailed Answer: Thanks for the query. You can safely use the spray even for 3 month period. But what I wanted to convey was that, sprays give improvement to the part of obstruction which is because of nose and adenoid par is not that affected. Sir, Please guide me about surgery of adenoids. Is it an endoscopic surgery .what will be the time period for hospitalization and by which time he will be normal. Please suggest me comprehensive procedure and good hospital for operations. thank you.

Brief Answer: Coblation assisted endoscopic surgery Detailed Answer: Dear, Adenoid surgery is done under general anaesthesia with no external scar. It can be done under endoscopic vision as well as without endoscope but endoscopic gives better clearance. Time period- we do it under day care (patient comes in the morning and is back by the evening). We do it coblation assisted and this method provides a bloodless way of removing adenoid under vision. Hospital, I do not have an idea about your place but in Amritsar, our centre is the only centre in Amritsar for bloodless ENT surgery. Feel free to ask another query, Regards
