What Is The Survival Rate Of Liver Cirrhosis?

Question: My father is cirrohsis pateint from last last 5 years and Diabetic patient.. treatment was going good for cirrohsis and for diabetes using insulin from last three years..but on 17-6-2014 diagnosed with liver cancer..HCC 7.8*6.4*6.3mm..my whole family is depressed.. ..doctor advised for TACE because surgery is not possible due to cirrohsis and we can not do liver transplant ..meanwhile doctor has advised for taking soranib 200 mg..so my father is taking soranib 200 mg from last 15 days and feeling so week,loss of appetite,nausea,B.P around 150..red rashes in hand..i am totally confused with this disease dont know what is going on..please sir suggest me what should we do??as there is only 1 option TACE..should we proceed with TACE ??how painful it is??how many times we should do TACE procedure..whether TACE have any problem with cirrohsis and diabetes??what would be survival rate??sir i am attaching blood test,endoscopy and CT scan report for your investigation..please help me

Hi dear,

I have seen reports of your father. His liver function tests are well.


Further you may do a biopsy from the left lobe of liver to know the severity/ grading of cirrhosis, but it is not absolutely necessary. He has mild to moderate risk in liver surgery as per his reports. now as the tumor involves only segment 6 and 7, so surgery is possible.

You need a good hepato-biliary surgeon, who can look at the CT images and decide about surgery. Best place I know for such procedures is SG PGI XXXXXXX There people are doing liver resection and even transplantation in cheap/ affordable rates. You have not included the prescription of surgeon who denied surgery.

TACE is trans arterial chemo-embolization. By this procedure we try to decrease the size of tumor. This helps in surgery and also decrease the symptoms. But cost is a factor.

The problems he is facing now are side effects of sorafenib. The full dose of sorafenib is 400 mg twice daily. It is a good drug. But if possible surgery is best.

Liver transplant is itself a big deal, which needs face to face counselling regarding many issues including liver donation/ financial.

1. How many times TACE procedure would be done and gap between procedure?
2. how much time it will take for TACE, i mean hospital stay and test??after procedure follow up??as i am from XXXXXXX and have to do TACE from XXXXXXX .

The question paper in my exams had 6 questions. Your exam is bigger.
I will try to answer.
1. It is decided by your aim i.e. u want only reduction in size or u want to get maximum benefit. Gap is to be decided by the doctor concerned and patients condition.
2. It is similar to cardiac angiography and angioplasty. approx. 1 to 1.5 hrs for procedure and average 2 days of stay, but it may increase depending on patients condition.
3. In expert hands the procedure is safe and complications are less. chances of mortality are less than cardiac angioplasty.
4. There may be complications, but usually benefits are more.
5. Depends on city, hospital and consultant.

1. not necessarily.
2. no body can comment on survival. its just expected survival which depends on many factors. surgery will be a step toward complete cure, but cirrhosis is a risk factor for recurrent HCC. for stage 2 patients, 5 year survival is expected to be 35%. definitely less for tace or sorafenib.

sir my last question is just want to know about before and after TACE procedure ..
i mean how TACE would be like after CT scan/ultrasound ?and complete bed rest days after that procedure and whether is there any complication in case of Diabetic patient(papa is taking insulin from last three years for it)..

Around 2 days bed rest is usually sufficient.

Do't worry about diabetes, the hospital will take care of it, and also the complications if there are any. usually minor complications only.
