A child of 4 months should pass urine at least 6 times in a period of 24 hours. No problems if the child passes more.

On the other hand it is normal for the child to pass stool ten times a day or even once in 10 days. A child may pass stool after each feed. The entire range is normal and nothing needs to be done so long as the stool is not hard or pellet like. The mentioned details are in context of a breast fed baby. In case of a formula fed baby, there are chances of infection occurring and hence sudden change in stool pattern needs to be looked out for.
