Brief Answer:
diabetes, hypertension, dementia

Detailed Answer:
Hello,

metformin, glyburide and insulin are for diabetes.
Hydrochlorothiazide and amlodipine are for hypertension.
Donepezil is for dementia.

Forgetting donepezil won't have any serious consequences.

Forgetting the rest of the drugs may lead to either hyperglycemia (if one of the antidiabetics is not taken) or to a high blood pressure (for the rest of the drugs).

Regarding insulin timing: you haven't specified what kind of insulin you use, but I suppose it's a long acting one like glargine or detemir. If this is your case then it wouldn't do a big difference if you take them a couple of hours later but it's better for glucose control if you take it at the same time each day. Also if it's a long acting insulin, there is no reason to take it 2 hours after meals. It can be taken at any time during the day but you have to make sure it's the about the same time every day.

I hope I've addressed your concerns.
