Question: Hi doctor,

My Father is diabetic for 25 years, i have written all the details in medical history.
Also i am attaching his latest reports. Kindly see and advise .

dear XXXX, Hi! your father seems to be doing good as far as the blood reports are concerned. you have not mentioned any specific complaint or query.
Few things I noticed in the reports.
1. High TG
2. High Potassium
3. high TSH
As per my advise, he can continue the same treatment for diabetes and hypertension except Tab Telma CT. It should be changed to cilindipine or prazopressin plus chlorthalidone can be continued. or any other diuretic can be added.
to make it simple for you, you can change to tab CTD 12.5 mg and tab cilacar 10 mg in place of telma CT.
Another thing is High TG, so along with ecosprin AV, you must add Saroglitazar 4 mg. Discuss about this medication with his treating doctor regarding changes in dosage or switch over to other medication. Until you consult your doctor continue same medication.
for High TSH , which is actually borderline, if he is symptomatic eg constipation, weight gain, dull, drowsy, tired, depressed, loss of appetite, then we may treat it or wait and watch for 3 months and repeat it to see if it is rising.

Few queries regarding medicines suggested/continued:
1. What is the dosage recommended for CTD and cilacar (OD/BD ??). What is the purpose of these medicines?
2. Is Concor 5mg ok to continue ?
3. dosage for saroglitazar?
4. My Father also has gastric trouble (too much gas pass and frequent loose motions). So is Razo D good to continue ?
3. I am little confused for what all medicines to take now. I would really appreciate if you can finally share all the medicines/dosage, my father should take overall along with the purpose?

Dear ma'm, let me tell you more clearly.
1. Tab cilacar is 10 mg once a day and CTD is 12.5 mg once a day .
both are antihypertensives. CTD was already going along with Telma. so I asked you to continue it. telma I changed to cilacar as potassium was high and ACE- I ie telma is not advised with high potassium.Discuss about these changes with your treating doctor.
2. concor is fine , he can continue.
3. saroglitazar ie lipaglyn 4 mg once a day for high TG.
4. razo D is good. but for loose motions you can give him sporolac or enterogermina ampoules orally.
5. rest he can continue same.
