Question: what is the difference between hydrochlorothiazide and chlorthalidone......i am currently taking HTZ and experiencing gout pain......is one safer than the other?

Both Hydrochlorothiazide and Chlorthalidone belong to similar class of medication. They have similar mechanism of action and side effect profile.
There is no extensive research available to prove that one medication should be preferred over other. However, the limited data currently available shows that Chlorthalidone is more effective diuretic in reducing blood pressure and protecting heart compared to Hydrochlorothiazide. Both these medication increases risk of gout.
If you are having episodes of gout while taking Hydrochlorothiazide, it would be better to another class of diuretic medication like loop diuretics (which include medications like Lasix) which have shown to reduce the incidence of gout.
