Brief Answer: Circumcision may be needed Detailed Answer: Hi, You have what we term 'phimosis' which is a medical term where the opening in the prepuce gets to be too narrow and inelastic and the glans penis which is the pinkish-red bulbous end of the normal phallus cannot be seen anymore. This happens commonly due to pent up secretions behind the foreskin not having been removed for a long time, and they in turn get infected, and this would explain the white discharge coming from the opening at the end. On quite a few occasions, we see this condition happening in diabetic patients, especially at a time when the sugar levels are not controlled, and once they see a physician and get the blood sugar levels back under control, the condition rapidly improves, and the skin can slide back and forth as earlier. In patients who are not diabetic, we recommend circumcision if the phimosis is really tight, and the surgery is a simple procedure of removing the foreskin alone and can be easily done as an outpatient procedure under local anesthesia. This will ensure the secretions get removed completely, and your personal hygiene measures can be effective. Hope I have been able to help you.
