Question: Hi,

I am writing in regards to Erectile Dysfunction. I have been experiencing it from a long time but it has been manageable so kept on ignoring it. I still have a strong libido.

I had performance anxiety even since I was in college. It eased of a bit when I got into a stable relationship with someone.

However, after my wedding to someone else. I have been in a troubled relationship from last 5 years and sex activities have been few. The stress/anxiety level has been very high.

My alcohol intake has increased in the last five years not because of depression but in general. i.e. once or twice a week. Its mostly because of the work culture in my IT company. However the alcohol quality is always top notch (if that helps)

Minor asthma was developed in the same years from sever winter cough etc.

However, I still feel fit enough considering that I completed a rigorous trekking of more than 30 Kms to Roop kund which is considered a mid level trek. I also dont feel a problem walking long distances. However running or really heavy physical exercise makes me go out of breadth.

I still feel natural erection in the morning and during foreplay extra.

Even during intercourse, i feel fine as long as the intercourse is going on but if I take a break for a few seconds, I loose the erection.

Now that my relationship is back on track I would like to know if there is a quick short term solution as well as a long term cure.

In treatment Erectile dysfunction (ED) first step is to identify cause of the problem. Erectile dysfunction can be a result of a psychological or physical problem.

Psychological causes: Most common cause in young patients like you. These include performance anxiety, emotional weakness, inferiority complex, depression, addiction (alcohol/smoking), stress and emotional trauma. Men with psychological problem usually have normal nocturnal erections and erections upon awakening, whereas men with physical problem often do not.

Man with psychological ED may have good physical stamina like in your case.

I will describe some simple measures which you can incorporate and these should help you:
-First of all, quit smoking and alcohol (if you are taking any currently).
-Eat a healthy diet rich in vitamins, minerals and antioxidants: fruits, milk and eggs
-Exercise, particularly aerobic exercise (like: Jogging, skipping and cycling) for atleast 30 minutes daily. It will increase blood flow all over body and will reduce stress.
-Considering your history your problem appear psychological to me. But it is better to get evaluated for diabetes, high blood pressure, heart or thyroid conditions, poor circulation, penile venous incompetency and neurological problem as these issues affect erection. If some of these tests are abnormal, then treatment may be needed for concerned problem.
-As you mention performance anxiety and depression in past life. These are one of the common reason for erection problem in young population. To manage them start yoga and meditation after learning from nearby specialist.

If there is no improvement in performance anxiety and depression with yoga/meditation then in person evaluation and examination by psychiatrist may be necessary in your case to treat psychological causes of ED.

Thanks for the detailed response.

I understand the suggestions you have given.

Do you think that as a short term solution it makes sense to try the sidenafil piils.

I have a local doctor i talked to sometime back who suggested me these but He suggested it as a short term thing and not something to pursue as a solution.
