Brief Answer:
Could be Rheumatoid arthritis

Detailed Answer:
Dear Friend.
You need to understand that high ESR of this level is not a classical feature of gout.
Also his uric acid levels are normal. We should reconsider the diagnosis of Joint pains.
Tell me which join ts are affected with pain?
Any pain in small joints of hand?
Any morning stiffness?

I am considering the diagnosis of Rheumatoid Arthritis because in that condition ESR can be so high.
If you have his any other old investigations too please attach them.
Stay Healthy.
