Question: Hi Dr. XXXXXXX Greif my name is XXXX I was having severe abdominal pain, back and flank pain and severe pressure when I urinated. They would not give me an appointment but the had me take a urine test and no one called me back. Then I ended up in the ER with more pain and inability to void urine. My test results were as follows:
WBC'S, UA/HPF 0 - 5 /HPF >25
RBC, URINE HPF 0 - 3 /HPF 26-50
BACTERIA, URINE HPF None /HPF Few
SQUAMOUS EPITHELIAL CELLS, URINE SED, AUTOMATED COUNT, QUAL None /HPF Few
MUCOUS, UR SED, QL, AUTOMATED COUNT None /HPF Present
AMORPHOUS CRYSTALS, URINE SEDIMENT, AUTOMATED COUNT, QUAL None /HPF Few
GLUCOSE, UA Negative mg/dL <30 (Neg)
KETONES, UA Negative mg/dL <10 (Neg)
SPECIFIC GRAVITY, UA 1.005 - 1.030 1.017

Welcome to HCM. I am Dr Anshul Varshney. I have read your query in detail. I understand your concern.

Your clinical symptoms and Urine report is suggestive of Urinary Tract Infection that might have been precipitated by Stones in the urinary tract that too particularly in Ureter.

Presence of WBC, RBC nad Protein in urine with your clinical presentation points towards this only.

However, it is important to rule out Nephritic Syndrome.

I would like to know following things:

1. Since when are you having this problem?
2. Any fever?
3. Any swelling over the body specially in peri orbital area?
4. How is your Urine Output.
5. Any history of hypertension?

This particular episode began about 3 days before I began to seek treatment ( I have had said symptoms before) I did have an on an off fever I would have a fever of 100 then a few hours later I was freezing and my fever was 96. I did have swelling in my vaginal area and was unable to void urine. That is what brought me to the ER. I always feel like I have to urinate but not much comes out at all and I have never had hypertension.

In such situation, i would advise you to get further investigated and treated as it seems to be due to Urinary Tract infection.

I would advise you following:
1. Urine culture and sensitivity
2. Ultrasound KUB.

You need to be started on antibiotics immediately.
So, get proper treatment and treat your nearest health center.

Does this mean I still have protein in my urine?

The report you have mentioned previously has WBC and RBC with Protein in urine.
This can occur in severe urinary tract infection.
Glucose, ketones are normal.
