Question: Hi I've already asked this but wanted to follow up: About 5 weeks ago had a sore throat, went to Dr and was diagnosed with Strep and took medication for it. After 2 weeks still tightness in throat. Started getting tingling in my fingers and toes. About a week later stiffness in jaw, difficulty opening jaw wide, tingling in face. This week my legs mainly my thighs have been sore and my legs feel really heavy when i walk. Also my back has gotten sore as if I have difficulty standing up straight. Also I feel shortness of breath with minimal physical activity.I'm at my desk now feeling like I'm breathing hard. just sitting at my desk. I walked up the street today and could barely breath. I've been to Dr and had EKG, Chest X-Ray, Blood tests to rule out vitamin deficiencies and other common illnesses. Gone to ENT and they put a camera in my throat and didn't find anything. Suggested it could be related to heartburn but I've never had it before and I am not convinced of this being the answer. The past two days extremely weak and feels difficult to take a full breath. If I run even a block I feel extremely winded. I'm feeling worse today feeling like I can barely take a breath, swallowing difficulty has been going on for about 3 weeks. Food feeling like its getting stuck in my throat. Extremely weak at night, I haven't been able to sleep much and after sleeping an hour am waking up out of breath. My dr doesn't think I need to see a neurologist but I almost felt like going back to ER or Urgent Care several times this week but I'm not sure what they would do. Could anxiety cause all of these symptoms?

Brief Answer: My reply is below. Detailed Answer: Hi, Thank you for getting back. I have noted your symptoms, and they point to a neurological problem. Tingling, weakness, tiredness, difficulty in swallowing, etc point to a problem with the nerves. You need to consult a neurologist. After detailed clinical neurological examination, the doctor would be able to come to a conclusion. He may order tests such as nerve conduction studies or MRI brain and spine to confirm the diagnosis. I hope my answer helps. Please get back if you have any follow up queries or if you require any additional information. Thanks are there any diseases that can cause such fatigue over 8 weeks in such a rapid stage? Also, are autoimmune diseases hereditary? Grandfather had mycinia gravies, grandmother Parkinson's, and my mom Sarquiodisis. Does that put me at a greater chance of getting an autoimmune disease? Could something as simple as an under/over active thyroid cause this?

Answered by Dr. Sudhir Kumar (9 hours later)
Brief Answer: Thank you for getting back. Detailed Answer: Yes, post-infectious syndromes commonly cause fatigue. However, it is more common after viral infections, and less common after bacterial infections. Also, you are more prone to auto-immune diseases in view of a positive family history. Basic screening for auto-immune diseases would include ANA profile, C-reactive protein, Rheumatoid factor, ESR, etc (all blood tests). Best wishes, Dr Sudhir Kumar MD DM (Neurology)

Brief Answer: My reply is below. Detailed Answer: Thank you for getting back. Taking an anti-anxiety medication such as clonazepam would help with sleep as well as any anxiety and breathing problem. Betahistine tablets would help reduce the dizziness. Breathing problem should be investigated in the meanwhile. Best wishes, Dr Sudhir Kumar MD DM (Neurology)

Are there any other tests you would recommend I have done? At night it feels as if I can't breathe properly through my nose and that I'm breathing through my mouth all night. I wake up with an extreme dry throat and tongue. Feels as if I can't swallow properly.

Ok, I'll ask the Dr about that. Sorry for so many questions shortness of breath appears to be getting worse my stomach feels very tight and I can't breathe well through my nose. Mostly breathing through my mouth all day which I think is causing the dry mouth issues as well.

Got Pulmonary function testing back and all looked okay. Still my legs feel very weak and still shortness of breath and tightness in chest/difficulty swallowing. The Pulmonologist thinks that its all anxiety as well. Is it possible anxieties can cause all of these worsening symptoms all of the time?
