What Does This Urinalysis Report Indicate?

Question: Hi, these are my mother's urinalysis values that are out of range or not normal:
>moderate blood in urine
>RBCs in urine 3 - 4
>WBCs 10 - 14
>moderate mucous in urine
>trace squamous cells
>leukocyte esterase-small amount
>trace bacteria
Also, her microalbumin/creatinine ratio is OK (22), although her microalbumin is high (2.3)

UTI is very likely. But a scan is a must.

Hello XXXX and welcome to HCM.
As an Urologist, i must advise you, that the urine reports, suggest urinary infection.
UTI (urinary infection), is confirmed by doing an urine culture.
As there's blood in the urine, it needs an ultrasound scan of abdomen.
She also needs to have blood creatinine, sugar,platelets,LFT(liver function test).
She's advised to get examined by an Urologist.
After initial reports are available,a decision on whether a CT-IVU,is indicated.
Kindly mention her other medication, so a proper advice may be given.

Hi Dr. Mangat,

Thank you for your response. I understand a UTI is very likely, and that you recommend a scan. Just to let you know my mother's values of the following:

blood creatinine- 0.7 mg/dL
blood glucose- 80 mg/dL
platelets-337 k/cu mm

You also recommend a Liver Function Test.

The only medication she takes is Losartan (Cozaar). My mother has always been VERY sensitive to medications, drugs and medical procedures.

I noticed that she is currently having several symptoms that are listed as possible side effects for Losartan: bloody urine (only microscopic), possible UTI, confusion, depression, frequent urge to urinate, lower back or side pain, unusual tiredness or weakness and weight gain. Previously, she was taking another BP med, which gave her tremor of the jaw, so switching back to that is not an option. My mother wants to get off her BP med entirely, and switch to a natural remedy that has good results, in hopes that the side effects will go away (if they are being caused by the drug).

Before my mother gets more tests (radiation) and drugs (antibiotics, possible dye) that may hurt her further, she wants to try getting off the BP med, and to switch to a natural remedy. Then she wants to get tested again, to see if she still has RBCs and WBCs in her urine, and to see if her symptoms go away.

I understand the importance of getting further tests for my mother, in case her condition is serious.

I know that BP medication is tricky to get off of, and that she needs to be under a doctor's supervision. But, can you give me an overview of how we can go about weaning my mother off her BP med?

Hi again.
You got a proper advice, regarding urine report.
An ultrasound scan is without radiation or XXXXXXX
Only if there's any cause for the blood seen in this scan, is a CT / MRI indicated.
That comes as a next step only, if indicated.
Regarding anti-hypertensive medication, you'll have to consult a physician, and it's not related to her urinary infection.

Thank you for the valuable information about my questions.

You mentioned that BP is not related to UTI. That may be true, but Losartan, the drug, has been linked to UTI. Sixteen percent of the 1,513 participants of the RENAAL study reported experiencing a urinary tract infection. Whatever you make of this, I thought I would share this with you.

I still maintain, B.P. isn't the cause for UTI.
It may be associated only, in patients having B.P.

I agree that BP may not cause UTI, but Losartan's side effects include UTI. Losartan itself may cause UTI.

large study has very minor associated UTI, in patients on Losartan.

Hi again.
For your reference sake, a peer reviewed study of FDA, out of 11,856 patients on Losartan, 270 patients had associated UTI, published on 27.9.15.
These patients also were on treatment for Hypothyroidism, arthritis,Hypertension,etc, and above 60 years age.
All symptomatic patients were cured after treatment(100 %).
Hope this clears your doubts.
