What Does This Upper Endoscopy Indicate?

Question: Hello,I had an endoscopy done over 2 months ago and this is how it reads under endoscopy findings:
1- Location [stomach body] the Finding was Gastritis
2-Location[stomach antrum ] the Finding was Gastritis Pronto Dry Test Positive
I was given 5+7 days treatment consisting of:
A:PANTOZOL 40 mg[ Pantoprazole sodium sesquihydrate]
B:ERACID 500 mg [clarithromycin]
C:FRAGYL 500 mg
Now after finishing the treatment my condition did not improves,actually I still feel food in my throat when I eat certain foods,I still feel full even I ate a little,and now I have a mild pain in my upper stomach along with discomfort in eating and loss of appetite
Please Help

I am sorry for the situation you are in.

At this point, I advise to repeat upper endoscopy to evaluate your situation better. I also advise to run biopsy of the stomach for H.pylori, which might be also another cause to lack of improvement.

Abdominal ultrasound is also recommended to exclude cholecyst problems. As per medication, if current therapy did not succeed, then, should change therapy and add Levofloxacine.

Next, as the therapy contains antibiotics, they can impair the intestinal flora leading to disbacteriosis or fungal infection. I advise to add probiotic at least until determining fungal infection, for which anti-fungal drug is recommended.

Right now I do not have the resources to have the endoscopy repeated.
would you please answer me about your opinion in the last endoscopy findings and the medication given?
Also please answer as why I have mild pain in my stomach after completing the medication?
Also please answer me about I still feel full when I eat a little and I feel the food is in my throat?

Hi back,

Your last endoscopy is showing :
1. Inflammation of the mucosa located in the stomach's body : gastritis
2. Inflammation of the mucosa located in the stomach's antrum: gastritis also. Antrum is the part of the stomach in conjuction to pylorum (part of intestines).

As the Pronto Dry Test resulted positive, it means that H.pylori is present and most probably the causative to gastritis. That's why the doctor prescribed the golden therapy to eradicated H.pylori infection and related gastritis.

Pantoprazole will help reduce the acidity of the stomach, thus, alleviating gastritis symptoms.

Eracid and Flagyl are antibiotics to eradicate H.pylori infection.

The mild pain you feel after completing the medication is due to:
- possible disbacteriosis caused by use of the antibiotics
- possible fungal infection of the intestines (candidosis)
- possible cholecyst inflammation or sludge

Please bare in mind that you have completed only 1 cycle of the full therapy. Another course of medications is needed to completely cure H.pylori and associated gastritis.

So, the first course of medications might have not been able to treat your gastritis and H.pylori, that's why you have having the sensation of being full and feeling the food bing in your throat.

That's why I have asked to repeat at least abdominal ultrasound to evaluate the status of cholecyst and to run another course of medications (pantoprazole, flagyl and levofloxacine this time).

Hello,thanks for the reply
I did the endoscopy overseas and could not do the test here for lack of insurance ,that is why I have asked for your kindly assistance in diagnosing what could the problem.
I also noticed that during the course of treatment I had 4-5 bowel movements but at the end of treatment it decreased 2 times a day and I started having diarrhea,what could the meaning of all this?
When I eat cooked healthy food and eats soup,I do not feel the mild pain in my abdomen anymore and do not feel the food is stuck in my throat,only certain foods make feel otherwise like cookies,wafers,bread etc...,what could be the situation here?

I am sorry for your insurance problems and for any other health problem you are facing.

The changes you noticed in your bowel movements during the therapy is due to antibiotics. As mentioned earlier, they change the intestinal flora, leading to changes in bowel movements and diarrhea (or loose stool).

Healthy cooked food are prepare with caution and proper hygienic measures. They do not contain other additives or preservatives neither. Cookies, wafers, bread, etc. (prepared outside home), they do contain additives, preservatives, colorings and other ingredients that might :
- change (lower) pH of the stomach; thus, aggravating/triggering gastritis
- increase the gas passing through the bowel, thus, giving the sensation of being bloated and mild pain
- if your cholecyst is also having sludge or inflammation; such foods will trigger the pain due to inducing bile liquid production and secretion while cholecyst is inflamed. This will also cause the symptoms you are experiencing.

I restarted another course of treatment as you suggested,But in case at the end of second treatment I still have the same symptoms[ feeling full,feeling food in throat,etc...} What could be the situation here?
How to check for symptoms if I have Candidodis or an inflammed Cholecyst?

With regards to Candidosis, should culture the stool for candida or culture oral swab for candida.

Inflamed Cholecyst is diagnosed with abdominal ultrasound and liver function tests.

Then, should revise everything we are doing to find out what is going wrong. So, apart medications and excluding candidosis, cholecyst and pancreas problems (as mentioned earlier), following a strict diet and lifestyle regimen matters.

So, it is important to:
- avoid coffee, chococate, spicy, chillies, etc.
- sleep with raised head (from shoulders) with 2 more pillows
- have your dinner at least 5 hours prior to going to bed
- do not lay down immediately after eating
- should start pancreatic enzymes to help digestion

At the end, should make sure everything is ok (as mentioned) before going to more specific steps.
