Question: These r the observation higlighted in usg abdomen of my father
Observations
Right kidney shows gross pelvicalcalyceal system dilation.
Right renal parenchyma is thinned out.
No calculus seen
Right uretor is not visualize
Prostate is enlarged in size, measures 66gms, shows mildly heterogeneous echotexture.

Imp
1. Gross hydronephrosis- right side(? Congenital puj obstruction)
2. Grade II prostatomegaly
3 significant post void residual urine

I have gone through your query. I assume that your father's age is on the higher side. The parameters like blood creatinine level needs to be checked in order to assess the degree of damage already caused to the right kidney. If his kidney function seems to be fine I would really not get bothered since the damage due to obstruction should have already been significant. On the other hand, if the functional damage is significant, it remains to be evaluated if there is significance of surgical intervention to relieve the PUJ obstruction.

The second part of the problem involves the prostatomegaly. Medical management is possible wherein progression of the condition is limited. But if the urinary urge, frequency, or episodes of urinary tract infection, are significant enough, surgery would need to be considered. There are less invasive methods like TURP (trans urethral resection of prostate) also available.

Consult a urologist for further treatment.

The creatinine level is elevated. You need to visit a nephrologist who would possibly do a DTPA and/or DMSA Scan and evaluate the structural and functional condition of the affected kidney. Based on the findings he would be able to suggest if surgery could help.
