What Does This Ultrasound Report Indicate?

Question: My health situation:

Age:45
Sex:Female
Height: 165cm
Weight: 58 kg (Notice weight gained from 52 kg to 58 kg in the last 12 months)
Marital Status: Never married, do not have any kids. No sex experience (due to religion).
Occupation: IT systems analyst
Monthly period: every 28 days, usually it comes on time, only once in a while it is early/late by one day. Each period lasts for approx. 5 days. The amount is normal except on 2nd day which has a bit more blood.
My last period: Started on 17th May and ended on 22nd May.
My next period: Expected to start on 13th or 14th of Jun (ie. 3 or 4 days from today)

Health issue:
* bloating (my tummy is big as if I am 4-month pregnant);
* easily feel full after meal;
* constipation (having poo once every 4 days with Type2 or Type3 pool according to Bristol Stool Chart,
* feeling occasionally dull pain & pressure inside lower abdomen sometimes on the left, sometimes in middle, sometimes on the right. such feeling occurs a few times a day, each time last for less than 1 second. Sometimes I may not notice the dull pain / slight pressure in it unless I pay attention to it. I am sure the pain/pressure comes from inside and not outside of my tummy.
* does not feel any pain during pelvic exam by doctor

Follow-up Tests
I saw one gynaecologist today who suggested me to have CA-125 blood test and CT scan.

I would like to know your advice on what other tests required before determining what type of surgery & treatment I need? What about transvaginal ultrasound or MRI scan?

And for CA-125 blood test, should I wait until my next period is over?

Multiple fibroids

Hi dear,
I have gone through your question and understand your concerns.
I have reviewed your reports.
You have multiple fibroids in the uterus, which is the cause of the abdominal pressure you are feeling.
There is no ovarian and adenexal mass, and hence there is absolutely no need of CA 125 levels.
No MRI scan or CT scan is required
You can undergo myomectomy or hysterectomy for the fibroids.

Thanks for your reply. However, referring to the details of the report, shouldn't more scan & tests are needed because of overlying bowel gas obscure some organs?

Hi dear, bowel gases do not obscure uterus and its pathology.
Hence multiple fibroids are correctly diagnosed on ultrasound.

Referring to the report, overlying bowel gas obscured 2 organs. Shouldn't those 2 organs need further check? Kindly please r

Hi dear, ovaries obsured by bowel gas are usually unremarkable.
I will suggest you to get a repeat ultrasound pelvis empty stomach, to assess the ovaries.
The tail of pancreas will be normal otherwise.

what kind of ultrasound do you recoommend? Lower transabdomen ultrasound or transvaginal ultrasound ?
