What Does This Transvaginal Ultrasound Indicate?

default
Posted on Sat, 8 Nov 2014
Question: Back in July I went to the ER for pain in my abdomen. A transvaginal ultrasound was performed, I was about 4 days away from my expected period. They found a 2.7mm simple appearing functional or follicular cyst in my right ovary. A tiny volume of pelvic free fluid. Multiple additional small follicles bilaterally. Right and left ovaries measure 5.1 x 2.2 x 3.0 and 2.5 x 1.9 x 2.5cm. My endometrial stripe thickness was 17.9mm. My uterus measures 6.6 x 3.3 x 4.7. My myometrium is homogenous. Arterial and venous flow was confirmed and Doppler spectral analysis was unremarkable. Is all of this normal? Would any of this effect my fertility? I've had a cyst previously in 2011, and 2006 as well. My cycle on average is 34 days. I am overweight as well. Is a 34 day average period normal?

Brief Answer:
ultrasound is normal, get hormonal profile done.

Detailed Answer:
Hello ,
Thanks for contacting health care magic.

The uktrasound report mentioned is well with in the normal limits in comparision to the day of cycle you were on .

Infertility is a concern in women who have PCOS. They have irreg periods, hirsutism, and polycystic ovaries. Also women who are hypothyroid or have high prolactin levels can have difficulty conceiving.
as you mentioned about your weight, being overweight increases insulin resistance and is a deterrent for pregnancy.

consult your gynae and get a hormonal profile done. i am considering that you have already undergone hysterosalpingography to rule out any tubal blockage.
review with reports please,

regards,

Thank you for responding. I had a hormone profile done last year, and my prolactin level was high, and my FSH hormone was low. To my understanding, this also depends on what stage I would be at in my cycle? At that point my period was late by several days. This year I have only missed one period, and that coincided with going to the ER and having the pains I had. I have no tubal blockage.

Brief Answer:
get a fresh hormonal profile

Detailed Answer:
hello
Thanks for the follow up.

Well one year has been a long time already. with higher prolactin levels and low fsh levels , it already indicates irregularity in menses . ideally they should be done at day 2 of your menstrual cycle.
prolactin is one hormone which if high can cause decreased estrogen levels and thereby infertility.
i suggest repeat the hormonal profile , including thyroid, prolactin, serum progesterone , serum testosterone , and FSH AND LH as well. Also a sugar profile is mandatory .

in the meantime, maintain a menstrual calendar for next 3 months and observe your pattern . review with reports please.
regards,
Dr.Sameer Kumar
