Brief Answer:
Possible infection.Scrotal ultrasound is indicated.

Detailed Answer:
Hello, Sir.

I understand your concern.

Normal liquefication is around 20 min.Abnormally prolonged time indicates prostate gland defect or ejaculatory duct obstruction.

Motility is also grossly affected as reflected in increased Grade D sperms.

Occasional pus cells and macrophages largely indicate an unresolved infection in the genital tract. For this I advise you to have a scrotal ultrasound doppler and a complete urine analysis with culture and sensitivity testing.

These tests have accuracy in establishing the cause for low sperm motility and high liquefaction time.

Once the test results are out suitable antibiotics may be indicated to clear the infection.Only after this intervention, a repeat follow-up of Seminal analysis is indicated.

Yes, lifestyle changes have a huge impact on seminal parameters particularly motility.

Avoid a sedentary lifestyle.Regular physical exercise for 20 min/day is mandatory.

Avoid smoking and alcohol.Alcohol can grossly impair sperm motility.

Consume protein rich foods like soy, egg and fish on a regular basis.

Pomegranate and Apples are required on a daily basis.

Weight reduction can improve seminal parameters.

Avoid junk, fried and fatty foodstuff.

Drink 3-4 liters of water a day.

Post your further queries if any.

Thank you.
