Answered by Dr. Chintan Solanki (16 minutes later)
Brief Answer:
Azoospermia- no sperm production/transportation

Detailed Answer:
Hello,

Thanks for writing to us.

In semen analysis when sperm is absent(zero) it is called azoospermia.

There are lots of causes of it. But technically two reason are there.
One is there is not at all production of sperms in testis which leads to infertility.
Second is production is there, but sperm can not make their way in ejaculate due to blockage.

Treatment depend upon its cause.

Hope I have answered your query. If still any query there, I will be happy to answer. For that please attache semen analysis and other reports if there.

Regards,
Dr.Chintan Solanki

Brief Answer:
Detail work up required

Detailed Answer:
Hi,

Thanks for follow up.

I can understand your concern.
I have mentioned the two basic problems/ways responsible for no sperm in ejaculation.Treatment depends upon specific diagnosis of the problem.

So you need to consult doctor and should go through following.

To reach to diagnosis of your problem further investigations are required.
1. Detail history of medical problems, exposures, past surgery, medications, and family history is undertaken by a doctor(surgeon preferably) to help define causes of azoospermia.

2. Thorough physical examination is performed.

3. Blood tests are taken that include testosterone and follicle stimulating hormone (FSH), sugar and thyroid.

4. Repeat semen analysis after centrifugation of semen to find out sperms. If by chance small amount of sperm found out with this method,it suggests there is no production issue but chances of obstruction of the pathway of sperm from testis to penis.

However if still no sperms found out in repeat analysis , testis biopsy is the method to diagnose the cause.

Few of the causes of absent production are:(for your info only , do not go into detail as your treating doctor will take the judgement after above investigations)
-Primary testicular failure, Klinefelter syndrome
-Y chromosome microdeletions
-Genetic infertility due to abnormal chromosomes (karyotype)
-Secondary testicular failure, Kallman syndrome
-Unexplained gonadotropin deficiency
-Hypothalamic/pituitary tumor
-Hyperprolactinemia
-Pituitary suppression, drug induced (anabolic steroids, alcohol, glucocorticoids)
-Testosterone supplements
-Congenital adrenal hyperplasia
-Severe illness (cancer, kidney or liver failure)
-Diabetes mellitus
-Sickle cell anemia
-Sperm autoimmunity
-Pesticide/toxin exposure (including hot tubs and baths)
-Undescended testicles at birth
-Obstruction, congenital absence of the vas deferens (CAVD)
-Epididymitis
-Scrotal trauma or surgery

Causes of obstructive pathology includes:
blockage can be due to prior infection, surgery, prostatic cysts, injury or congenital absence of the vas deferens (CAVD)
Ejaculatory duct obstruction
Ejaculatory duct obstruction
Vasectomy

After reaching to diagnosis treatment can be done.
In case of absent production correction of the cause with medicines/hormones can be helpful.
In case of obstruction microsurgery is required to remove obstruction.

If you have good erection and ejaculation is there, you will be able to satisfy your wife in sex.Sperm is responsible for reproduction. It means if you need child and make your wife pregnant you have to find out the solution.

Hope I have answered your query, I will be glad to answer more.

regards,
Dr.Chintan Solanki.
