Question: my 19 year old daughter has a cholesterol of 235 and the LDL is 164. she is thin and has a healthy diet. is this concerning?

first, was it a 8h fasting lipid profile, if yes it's fine.
Secondly, we need to confirm this with a second test after 2 weeks.
Is there a family history of heart attacks, if yes then it becomes a priority to prevent heart attack.
First step, is to change the diet to strictly avoiding saturated fat, low oilyand junk food. Avoiding potato chips, fried packed food, avoiding Sweetened beverages etc. daily exercises even if not overweight, reducing sedentary time to less than 2 hrs in a day. we will wait till 4-6 months and if no satisfactory response then we need to put her on medicine like tab Atorvastatin 10 mg bedtime.
you can take one diet consultation as well with total fat intake is limited to 30 percent of total calories, saturated fat to 7 to 10 percent of total calories, and dietary cholesterol to less than 300 mg/day. Reply if you any doubts.

Yes, it was a fasting test 30 days from her previous one. She has started accutane but the first test was prior to beginning the drug and her cholesterol result was the same, before and afteraccutane. She exercises. There is no history of heart attacks and she has low blood pressure. I gave her the same advice about trying to change her diet but your specific percentages for fat is helpful. Hopefully with an aggressive change in diet (although she really does not eat poorly) and increased exercise she can change this. I had the dr run a thyroid check because my cholesterol used to hover around 200 and once i went on synthoid, my cholesterol immediately dropped. Her thyroid levels were normal however, so we're just not sure what would cause this. Can you ever have naturally high cholesterol?

Most of times this is a genetic problem due to deficiency of enzymes metabolising cholesterol. Accurate usually increases triglycerides only, not cholesterol. I guess she is not on other drugs.
I think she has got acnes as well, does she have menstrual irregularities, darkening of skin creases. once sugars should also be tested. PCOS should be ruled out in her by doing ultrasound pelvis, and hormonal Testing. Hormonal Testing is important as it can lead to all her problems.
If possible get her endocrinologist and gynaecologist consultation done as well because all these problems can be very well linked to hormonal changes.
