What Does This Lab Test For Dengue Fever Indicate?

Question: Hello Dr.

Please find attached Report for patient for dengue and other tests.As per Dr here, looks like viral....but fever went till 105 in last 2 days and now its normal after viral infection dose.

please suggest.

I have gone through your query. Quick rise of high temperature is a characteristic of dengue. Platelet count is diminishing and packed cell volume has decreased also. These are usual signs of dengue.

Take more fluids and have adequate rest. If high temperature persist hospitalization is necessary. Intravenous fluid administration will be needed in such case. Platelet count should be repeated every day. Usually platelets will decrease after 3rd day.

I have gone through the report. It is widal test for typhoid which is negative. If platelet count is going further low then you test for dengue igm and lepto igm test to rule out whether it is dengue or leptospirosis.
And if there is intermittent high fever and chills then peripheral smear should be done when fever is high to rule out any possibility of malaria. 4 hourly temperature should be recorded to get the graphical pattern of fever also.

You need bed rest and adequate fluid intake. Hospitalization is necessary in such case. Intravenous fluid administration is helpful also. You can also take chewable vitamin C , pomgranate and pappaya leaf extract for increasing platelet count. Once the count start increasing then nothing to worry.

Platelet count should be monitored 2 to 3 times daily. If it is decreasing below 15 then platelet infusion will be needed.

Black stool is a sign of Lower GI bleeding which is due to low platelet count. Patient should be hospitalized and be strictly in bed rest. Intravenous fluid should be given according to needs.

If platelet becomes lower than 20000 then platelet infusion should be considered. So platelet level should be monitored. If there is any increasing tendency in platelets then nothing to worry.

We also give doxycycline or crystalline penicillin injections to cover up leptosirosis also as both Dengue and leptospirosis have same clinical picture.

We have admitted the patient in the hospital. Blood platelet has been started inserting into the body. Are there any other precautions we need to take?

Also I wanted to ask can the patient be discharged from the hospital after getting the platelet count normal? Or is it advised to still get admitted for some more time?

Patient should be in bed rest till platelet goes up to 1 lakh. Other than that no precautions are needed. If she has any symptoms like severe head ache or chest discomfort report to your doctor immediately. Once platelet reach 1 lakh patient can be discharged.
