Brief Answer:
Insufficient information

Detailed Answer:
My dear,
Thanks for sending your query to us.

The report you have sent us is for the calculation of lens power to be put in the eye. There should be another paper where eye examination details are given, that report will inform us about details whether you have cataract or not.

Get back to us with relevant details so we can answer your query satisfactorily.

Awaiting.

But does the report says that both the eyes need powers for proper vision?

Brief Answer:
Inconclusive report

Detailed Answer:
My dear,
This report shows measurements of length and curvature of eyeball. This data does not indicate whether you have cataract or not. This data is applicable to all eyes. There has to be a separate sheet where amount of cataract and level of vision are mentioned. On the basis of that data suggestions about cataract can be done. This sheet is just measurements. That sheet which is missing will have your complaints and vision and findings of eye examination.

Since it's a old report I don't have the separate sheet. I am sure on this report you can atleast say whether the eyes are ok or not leave aside CATARACT.
