What Does This MRI Report Indicate?

Question: I went to clinic because of RUQ pressure that was randomly noticeable. Had an MRI. The clinic staff couldn't tell me anything other than give referral. MRI results showed the following: "Mildly echogenic and heterogeneous without focal hepatic abnormality. Liver is 17.1 cm in saggital diameter which is enlarged. Normal portal venous flow is demonstrayed" can you explain, set my expectations, and clarify the heterogeneous concern? Thank you.

Before answering your query i would like to give you a lay mans briefing about these terms;
Parenchymal echogenicity means the ability of the functional part of an organ to produce an image.Principal of MRI is that it splits water In cells which is capture by computer screen.In a normal person liver is made up of same parenchymal cells with equal activity.As activity levels are normal blood supply would be normal to every cell hence we can say that all cells in liver would be containing equal amount of water(Blood contain 75% water).So if a MRI is done on a normal person all cell will show spliting of equal amount of water hence the image formed on computer would be Homogenic(same texture).

Now lets come to your query
1)Significance of mild echogenic and heterogeneous parenchyma-As we grow old fat begins to accumulate in our liver medically termed as fatty liver.There are various grades of fatty liver however mild to moderate fatty liver is common in elderly above age of 40.
A heterogeneous liver can be caused by fatty liver changes, tumors or cirrhosis. MRI scans can detect the additional fatty structures in the liver, which appear on the scan as areas of lighter-colored tissue. Fatty liver is a reversible condition that can be brought on by bad diet or high alcohol consumption. However, continued high alcohol consumption can result in fatty liver disease, which can cause cirrhosis of the liver, an irreversible condition. Due to accumulation of fat in liver cells the splitting of water changes from cell to cell(some have accumulated more fat then other) leading to formation of a heterogeneous texture on MRI scans.

However as expalined earlier mild changes are associated with age related fat accumulation and are nothing to worry about.

2)Without focal hepatic abnormality-In case texture changes are due to problems like cirrhosis or tumour of liver there will be focal(particular) area involved.As tumour will be confine to a particular area only change in texture and echogenecity will be confined to that area only.

As per your reports don't panic its normal and changesa re just age reated.

Its clinically normal.

Hi again,
Normal liver size is about 15-16cms.
Your liver size is 17cm(Just due to accumulation of fat around liver).
Clinically its normal.
