What Does The Ultrasound Report Indicate?

Question: Im XXXX, aged 49 (DOB: 1st Feb 1965) and my first ultrasound clinic gynaec report on 29th Oct 2014 shows :
Real Time B Mode Ultrasonograph of the Pelvis Done Abdominally and Vaginally.
Uterus Position : AV - Size 95x54x48 mm
Endometrial Echoes - Thickened Echogenis - Thickeness 12 mm
Mymetrial Echo : Homogenous - Cervix Echo : Normal
Right Ovary : size 32x17x14 n - Vol 4.1cc

My 2nd ultrasound clinic gynaec report done today i.e., 18th Nov 2014 shows
Uterus position: AV, Size 95x45x62 mm - Endometrial Echoes :Thickness 12 TL mm
Myometrical Echo : Homogenous : Cervix Echo : Normal
Right ovary : size 28 x 16 mm
Left Ovary : size 23 x 20 mm - 1 Foll 17x14mm, 10mm
Impression: Enlarged uterus with 12 mm endometrium. Both ovaries appear normal: Left ovary shows dominant follicle
Im worried, please let me know what does the above results indicate

Hello dear,I understand your concern.I have seen your reports.

In my opinion your report is showing enlarged uterus with thickened endometrium.
The endometrium thickness depends on the
phase of the cycle.
There are two phases in menstrual cycle.
1) Follicular phase.- phase before ovulation.
2)Luteal phase-phase after ovulation.

Endometrium reaches maximum thickness in follicular phase.

At every menstrual cycle a group of follicules are recruited.Among which one follicule grows and gets ready for ovulation.That follicle is called dominant follicle.

The dominant follicle is seen before the ovulation and it collapses after ovulation.

So presence of dominant follicle is normal.
And endometrial thickness depends on phase of menstrual cycle.

Before telling about I need further information like-
1)How many children you have?
2)How are the cycles regular or irregular?
3) Do you have pain during menstruation?
4) Do you have any heavy bleeding?
5) Do you feel pelvic discomfort ?
6) Do you have backache?
7) Do you feel pain during intercourse?
8) Are you taking any treatment?

Kindly provide me the information and I am ready for further discussion.

Thank you Doctor your immediate reply. My answers to your questions :

1) I have two boys, eldest is 23 yrs (DOB: 19.02.91) and the 2nd is 22 years (DOB;20.1092)
2) My cycles were regular except from this year. In XXXXXXX Feb and Mar I had no periods, from April onwards I have been having my periods sometimes twice a month. On 8th Oct I got my periods only for two days and the bleeding was very heavy, something I experienced the first time in my life. This month, Nov 2014 I had my periods on the 4th and ended on 7th as doctor had given me tabs to stop the bleeding.
3) I used to have very severe stomach and knee pain during periods time, but for last 2 -3 months there is absolutely no pain at all.
4) Heavy bleeding was only in Oct 2014. The earlier months was the usual bleeding. From the time of maturity I used to get my periods at 8 - 10 days a months and after marriage it stopped to 6 days. Now gradually its reducing to 3 -4 days.
5) I get this itchy feeling most of the time.
6) I have never experienced any back pain during period time.
7) I am a divorcee since 1994. Got married in 1990 and separated from my husband in 1994. Ofcourse I did have a relationship in 2004 for 2 years.
8) Doctor has not started any treatment yet. Will be meeting the doctor tomorrow with my 2nd scan result.
Please let me know if there is anything serious...............i am most worried

Dear madam,

I got your information.

In my opinion as your cycles are irregular we cannot predict the ovulation and the phase of the cycle to say that thickened endometrium is due to follicular phase of the cycle.

But to rule out any problem associated with thickened endometrium we can go for a simple procedure called D&C.

Dominant follicle means nothing but you are ovulating.

If a patient comes to me with similar complaints I would do D&C(Dilatation & Curettage) and send the endometrial tissue for histological examination to rule out any abnormality associated with thickened endometrium.

Bulky uterus at this age might be due to a condition called adenomyosis where there is symmetric enlargement of uterus.

Adenomyosis presents with heavy bleeding during periods, pain during periods and enlargement of uterus.

If you have problematic periods with adenomyosis treatment might be hysterectomy.

Your condition is not serious and nothing to worry about that. And just relax.

Adenomyosis is mostly seen in women in 40s or 50s.

If no complaints and D&C report is normal and just an ultrasound finding of enlarged uterus is present you might just leave it.
