What Does The Semen Analysis Report Indicate?

Question: Please Doc, I abstained from Sex for few days before semen analysis at the lab... The result is as follows:

Time Produced: 2:46PM, Time Received at Lab: 2:50PM,
Liquefaction : after 30 minutes

Volume: 3mls
Appearance: clear
Viscosity: slightly viscous
Odour: Straw


Motility (Active): 40%
Motility (Sluggish): 40%


Total Spermatozoa count : L 2 Million/ml 20 - 300

The following was also observed from my semen analysis:

PUS Cells: 1-2
RBCs
Epithelial Cells: 2 - 3

Semen (Culture and sensitivity)
Culture Report INSIGNIFICANT GROWTH


1. My wife and I have been trying to get pregnant for 10 Years. We have tried IVF 3 times, One as sucessfull but lost it due to Fibroid, the fibroid has been removed now but my count is still low and no this result, i am lost of options in achieving pregnancy for my wife. is there any hope for me?

2. What does PUS Cells being 1-2/hpf mean?

3. What does Epithelial Cells being 2 - 3 mean?

4. What do they mean by Culture report INSIGNIFICANT GROWTH

5. What is your opinion of my Semen Analysis?

Hi,
Let's hope I answer all your questions.
Your pus cells and epithelial cells are within normal limits as they ought to be in most normal men. There should be no infection in the semen and your culture confirms that.
Your count is definitely on the very low side and as you must have been counseled, IVF with ICSI would be the best option.
To see if you can get a better semen in the future, you could try and get some basic hormonal evaluation including serum testosterone total, serum FSH, LH, prolactin. An ultrasound of scrotum with Doppler study if a clinical examination ever was suggestive of Varicocoele should be done.
Recently most Infertility centres are doing a test to evaluate DNA fragmentation by a novel technique called SCSA. This might be predictive of how successful future trials of IVF in your case would be.

Thank you for yours response, can i know what is the pissible cause of low sperm, i can recollect that it was not like this in my teens. Is thre any drugs to correct this?

Hi again,
As a matter of fact, the tests are to find the cause, and thereby hopefully treat the underlying condition. A progressive deterioration in semen quality with age could be due to a Varicocoele which is the commonest correctable anomaly in men. However not everyone improves, and those men with a higher FSH might not do very well.
