What Does The Following Semen Analysis Report Indicate?

Question: Hi Sir/Madam,
me and my wife are trying to conceive for the last 5 months without any success. please examine the attached my wife's medical report as suggest by local gyno and my semen analysis report.
please also consider below points while examining our case -
1) my wife has a monthly cycle of 34 days. and she gets her period quite regularly and accurately on 34th day.
2) but few days before period she used to experience pregnancy symptoms for the last 5 mnths since when we r trying.
3) however this month her 34th day was on 3rd Sep, 2015 when she did not get her period. Add to that she got heavy symptoms of vomiting, early morning sickness, headache, nausea. so we were sure she would be pregnant this time around and were waiting to do her home pregnancy test after 6 days more.
4) but on her 38th day today at 3.30 pm she got her period again.
Please analyse and suggest us if there is anything wrong.

I have attached my 2 semen analysis report. in both the reports the volume is quite less which was because of the performance pressure. my normal volume is much higher than the test volume.

She is currently taking below medicines as prescribed by our local gyno -
1) Ovalife
2)Folvite
3) ycyst - M

I can understand your concerns.

First, Please allow me to clear this;
Infertility is only labelled when a couple is unable to conceive after 1 year of unprotected intercourse. So technically your cases is not of infertility. And I definitely believe that your wife can conceive.

Now coming to your queries, I have gone through all your reports thoroughly. Your wife's ultrasound report shows polycystic ovarian disease. I feel this is the main cause behind inability to conceive. Her disturbed menstrual cycle is probably due to this.
In my opinion, she should continue taking metformin(ycyst-M). This drug treatment is known to improve fertility chances in many cases.

Other than this, I would suggest that she should go for an ovulation study. This will determine the status of ovulation, her follicle size and their time of rupture. Usually the follicle ruptures after the size of 20mm.
If there is any problem in ovulation, drug clomiphene will be needed to induce ovulation.
Beta HcG injections will help in follicular rupture.

Your semen analysis is a normal one. I am not worried much about it as you said volume is normal.

Other than this, she must undertake some lifestyle changes for PCOD like brisk walking for 30minutes a day, avoiding high sugar and fatty foods.

While you perform intercourse, it should be around day of ovulation. In your case it should be more between day 17-22 days of her cycle.
Keep a pillow under her buttocks after you ejaculate inside her so that semen is completely inside vagina and does spills out.

we will definitely go for the Ovulation Study test and will share the report with you. can you please give us a brief about this test as when and how this test to be done. what is the exact lab name of this test?

Can you please recommend any medicine to keep us interested during the ovulation time. due to performance pressure and regular intercourse during the ovulation period we both sometimes loose interest in intercourse and miss the most important day due to this.

so should we calculate her next ovulation time taking into account her 38 period cycle or the original 34 period cycle now? should we use a i-sure ovulation kit for better prediction.

Ovulation study, also known as follicular study, is simply done by ultrasound. It determines the size of ovarian follicles, there maturity and timing of rupture (sorry for bein technical. In simple words, it determines maturity of eggs in your wife's ovaries).

This may be a guide for further management.

Regarding intercourse and interest, I feel that your sexual activity has become more target oriented.
Sexual activity should be a pleasurable encounter.
Perform intercourse when you have the urges. Adopt different postures and places in your house. Create a nice ambience during sex. XXXXXXX your partner sometimes. This will rejuvenate your interest in those acts.

You can calculate ovulation day by her original 34 day cycle. Ovulation predictor kit is a nice idea to determine approximate time of ovulation.
But sex should not be a work which has to be completed. It is and should remain a pleasurable act. I would not recommend any drug remedies to generate interest. This will create more burden of remembering drugs and taking them.

I searched in internet abt follicular study and came to know that for this test we hv to do ultrasound multiple times in 1 menstruation cycle. So this seems to be a big effort. Should we go for this right now or should we opt this as an last chance after trying for 5-6 mnths more? Is this really such a difficult diagnosis to carry out as suggested over Internet?

Hello,

Sorry for late response.

You are right.For follicular study , you need to go every alternate day for ultrasound till mid of cycle.

It is cumbersome to get ultrasound every other day.you can postpone this investigation for 5-6 months.

I advise you to follow instructions like intercourse in mid cycle and keep pillow after intercourse.

Actually in technical term infertility is not able to conceive even one year of unprotected sex.so you should wait for another 7months before going for any investigations.
