What Does The Following Follicular Study Indicate?

Question: Hi,

I am trying to conceive for more than an year now. I have taken clomid 50 mg in the past for 5-6 months and my follicles have grown also as per the follicular study.
Last month, we went to an infertility specialist and she suggested we go for IUI but diagnosed a cyst and hormonal imbalance of LH and FSH. She prescribed Glycomet 100 to be taken continuously and ovral L for a month for cyst. After the cyst subsided in the last month she gave clomid 100(glycoment ongoing) and follicular study from 10th day. on 10th day a simple cyst measuring 29mm was seen and follicle size was as below:
day 10: RO: LARGEST 6.4 mm and 9.7mm, LO: same
Day 12: RO:Largest 9mm and 7 mm, LO: no change
Day 14: RO: 9mm and 8.2 mm, LO: no change
the cyst had collapsed.cyst was in RO. I am puzzled because my body has always responded to lower doses of clomid and this time with 100 mg on 14th day my follicle size is not good. Please advise what is the issue.

I read your query and I understand your concerns.

Following is my reply:

1) You have not responded to the stimuation as there was a cyst.

2) This simple cyst of 29 mm has possibly prevented other follicles from growing.

3) hence it is ideal to have ultrasound scan on day 2 of cycle before beginning stimualtion to rule out cyst. If no cyst then only go ahead with stimulation.

Sorry Glycomet given is 500mg.
In the beginning of the cycle on day2 and ultrasound was done and doctor ruled out any cyst and so they gave clomid 100mg.
I am unable to understand why the follicles are not growing and there is a cyst even when it was diagnosed in the beginning of the cycle that is no cyst.Please advise

Hi,

1) Glycomet should only be given if PCOS is suspected. If not it is unnecessary.

2) If there was no cyst on day 2 the present 29 mm cyst is likely to be a early grown follicle and not cyst.

3) But this is grown early. This happens when ovarian reserve is low.

4) Please let me know your FSH and AMH blood levels if done recently.

5) If possible let me know which city you live so that I can suggest a better doctor if i know.

Blood test was done on 2nd day of cycle. below is the reading
Estradiol(E2),Serum(CLIA)-46.39PG/ML
FSH:4.71 mIU/mL
LH: 3.25mIU/ml

Your hormonal profie seems to be normal. Get AMH also done next time on day 2.

You follicle has grown early and 29 mm size. This is not a cyst.
