What Does The Following Fetus Measurement Report Indicate?

default
Posted on Mon, 12 Jan 2015
Question: I am currently 36.5 weeks have been receiving growth scans since my 31st week and the Femur has been consistently measuring small and I have been worried about SKD.

My measurements at 31 weeks:

BPD:          80
HC:          293
AC:          265.5
FL:          56

My measurements at 33 weeks:

BPD:          86
HC:          315
AC:          282
FL:          60

My measurements at 35 weeks (or 34w + 5.5 days):

BPD:          89
HC:          317
AC:          294
FL:          63.9

It seems that it continues to measure smaller (by about 2 weeks each time) and I’m fearing my next scan as I am worried I will be told that the baby has a Skeletal Dysplasia. My fluid levels and the babies biophysical profile have been 8/8. I realize it’s also a soft marker for DS, however, I did the quad testing and additional Panorama testing for T21 and the NT scan at 12 weeks was perfect as well.

Can you tell me if you believe it could be a form of dwarfism? If you don’t believe it is, what makes you believe it isn’t?

Brief Answer:
This is not dwarfism or skeletal dysplasia

Detailed Answer:
Hi,

I read your query and I understand your concerns about reduced femur length.

Following is my reply:

1. Let me reassure you that, reduced femur length by about 2 weeks is not dwarfism or skeletal dysplasia.

2. This is just a normal ultrasound variation where in 2 weeks variation is expected and it does not mean dwarfism.

3. Ultrasound scan always has variation of about 2 weeks and not to worry.

4. Moreover your quad tests, Panorama and NT scan are also normal meaning there is nothing to worry about.

5. Hence dont worry as all is well.

Let me know if you have any more doubts.

Dr. Mahesh

Thank you. That is very reassuring.

When does achondroplasia usually present itself? Would it be obvious at the 35 week mark? Would the delay be much greater than 2 weeks? I am so worried to go for my next scan and be told there was no growth at all!!!

Brief Answer:
Proportional increase exists

Detailed Answer:
Hi

2 weeks lag is fine. But good news is that femoral length is increasing in each scan and is lagging by just 2 weeks. But is is increasing.

If it was dwarfism it would have been evident by 20 weeks itself and lag would be much greater.

There would have been no increase at all at current rate if there was dwarfism.

I don't think there is any reason to worry.

Thank you.

So, the fact that his head is ahead by a week and his legs are behind by 2 is completely within the normal ranges?

And if it has been a skeletal dysplasia it would have:
a) showed sooner
b) be off by more weeks
c) not shown any growth at all past 30 weeks?

Thanks for letting me know!
