What Does The Following ESR Value Indicate?

Question: Hi,
My ESR values are 1st hour 45mm and 2nd hour 70mm, i have tested this with couple of diagnostic centres the values are almost same. When i had cough 45 days back first time i took this test that time it was 1st hour 52mm and 2nd hr 84mm but after 1 week my cough was completely cured now i dont have any symptoms like cough, cold or fever byt my ESR is still high 1st hour 45mm and 2nd hour 70mm. My doctor suggested some other tests like fever profile, CBP and chest x-ray everything is fine except high ESR now my doctor saying high esr without any sympoms need not to be worried.

Any suggestions please.

Hi,

Thank you for your query. I can understand your concerns.
ESR is non-specific and not a good guide.You can go for C-reactive protein.
Combination of C-reactive protein & ESR measurements improves diagnostic sensitivity and specificity.
However raised ESR should prompt for further evaluation to exclude chronic infection , collagen disease , neoplasm and renal insufficiency.
Anemia itself may cause raised ESR. What is your Hemoglobin level?

Persistent high ESR

Detailed Answer:
Hi,

Thank you for your query. I can understand your concerns.

Your Hemoglobin (14.1 gm/dl) level is normal.
Normal ESR in adult male is 0-10 mm/hour.
One of the common cause of high ESR in XXXXXXX is tuberculosis, which may be occult.
Watch your weight. If you keep losing weight, go for TB gold (interferon gamma release assay/IGRA).

A plain x-ray chest may not show para tracheal lymph node enlargement clearly or subtle lung parenchymal lesion.

I suggest HRCT (High-resolution computed tomography)chest and USG (ultrasonography) abdomen to exclude any occult focus of TB, since elevation of ESR was detected following a febrile episode.
