What Does The CT Scan And MRI Report Indicate?

default
Posted on Tue, 21 Apr 2015
Question: I have a history of mpnst more than 4 years ago. The tumor was almost 1 cm and free margins were obtained with no chemo therapy needed. This tumor detection was accompanied by elevation of reactive protein. Few weeks ago i noticed a small lump near my left shoulder, it is also very small, hard and movable with no pain. My ct scan and mri done 8 months ago were completely clear. Following this small lump i ran a CRP test, and it is negative or non elevated at all. Does this excludes the possibility of a recurrent tumor, or i need to go for an early CT and MRi? I have consulted a general practiciN and an ortopedic surgeon and they both told me after examination to completely ignore it. Any advice? Thanks

Brief Answer:
Yes, it may need further evaluation

Detailed Answer:
Hi
Thanks for your query.

I understand your concern. You have not mentioned regarding the site of the MPNST. Was it near the left shoulder? This is one of the major determinant of our clinical decision.

If I assume it was away from the site of present swelling, then the probability of it being a recurrence is less but still present. Next important thing is the clinical examination. If the feeling is like a benign swelling like lipoma or cyst then we can observe, but as you said it is 'hard'. In that event I would like to get a FNAC done and rule out recurrence. CRP cannot rule out recurrence of MPNST.

Probably you should visit an oncologist rather than orthopedician or GP as he/she will be better equipped to take care of this.

Hope this helps. I will be available to answer further followup queries, if any.

Regards

The site of the mpnst site was left pectorial and it was deep. This new thing is more clse to the surface of the skin. Also it is not swelling, but i rather felt it when i slightly pressed through the skin. Worth mentioning that i also have an injury in my oeft shoulder due to excessive use of IPAD resulting from iritation of the shoulder muscles. Could this be related to this small lump? The lump is small, hard, and it moves freely when i press it. I will check with an oncologist either ways, and will ask to have an early CT and MRI as well.

Brief Answer:
Need to investigate

Detailed Answer:
It does not appear to be because of strain to shoulder. However, as the MPNST was also in shoulder region, I would like to do a proper examination and an FNAC. CT/MRI alone will not clinch the diagnosis.

Best wishes
