What Does Presence Of Epithelial Cells And Cloudy Urine Indicate?

Question: I went onto another medical website and had the following conversation with a doctor. I want a second opinion and I'm wondering what should I do from here, besides following the FODMAP diet. I do want to note that my mother has kidney problems, but she's on medication for it and has NEVER been hospitalized for anything related to her kidneys.

My Question to the doctor:
What's the likelihood this is kidney cancer? 27/F. I've noticed off and on, and especially in the last few days my urine is sometimes cloudy. I do take vitamins (multivitamin and a Vitamin C daily). I haven't been as hydrated as I should. I know that both of those things can contribute to cloudy urine. However, I've also had a bloated feeling on the left side of my back into my abdomen. I just thought it was pulled muscles from carrying my two year old on that side all of the time. I have no pain when I urinate, and no back or abdominal pain, just a "pressure/bloated/full" feeling. My appetite is normal and I don't have any unintentional weight loss. I've had this pressure feeling for MONTHS. At least 6...which has me even more worried that if it is kidney cancer, it's advanced. What's the chances that this is what's causing the cloudy urine and pressure feeling? I did the vinegar urine test and my urine did NOT clear up. I had an abdominal ultrasound for an unrelated cause in January - so 11 months ago - and it was normal. My last urine analysis was last month when I saw my OB/GYN and they just did the dipstick there in the office. Would that have shown blood in my urine?

I will say very low or none.
Cloudy urine can be caused by the dehydration commonly.
Also, it can be cloudy from presence of epithelial cells in the urinary tract or vagina.
Bloated feeling and pressure suggest the gas problem which is GI issue rather than kidney issue.
Ultrasound is sensitive to pick up any kidney cyst and you did not have one.
Urine dip test is also good to pick up and presence of blood or protein.
And I do not believe you have any family history of kidney cancer.
So, in general at your age this is a very low probability.
And you do not have any significant anemia, or weight loss.

My response:
Yes, no family history of any type of cancer. I remember I was in the ER for an unrelated issue 13 months ago and the urine test did show epithelial cells. The doctor didn't even mention it. And I went in to my regular doctor for a follow up after that and he looked over all of my test from the ER and didn't say a word about the epithelial cells. Ok, so where do I go from here?

Please watch your diet and eat lot of anti oxidants and healthy diet.
You should consider following low fermentable sugar diet ( FODMAP diet) to reduce your symptoms.

So I want a second opinion to his response, and I'm wondering if I need to see a doctor or what to do from here. I did just have a urine dipstick in my OB/GYN's office last month that was clear as far as I know. They just tested it there in the office, they didn't send it out to a lab or anything. Is it true that the urine dip in the office would've shown blood in my urine?. At that time I was also tested for STDs and that was clear as well.

You need not be worried as the previous doctor said..Presence of epithelial cells,cloudy urine doesn't necessarily indicate a serious pathology.

Considering your age,family history,negative urine dipstick test,normal abdominal ultrasound,it is highly unlikely to be a kidney cancer..

Yes,urine dipstick can find the presence of blood cells..If its positive,microscopic analysis is done to find the number of blood cells.

I would advice you to visit a gastroenterologist if you get the symptoms like bloating,fullness frequently .In mean time follow healthy diet(with plenty of oral fluids,encourage fiber rich diet etc) and active lifestyle..

Thanks. I just want to note that the ultrasound was 11 months ago to see if I had a swollen spleen (which i didnt) due to low platelets. Turns out I have ITP. Anyway, it was 11 months ago - isnt that a long time? Could a cancer taken place in the meantime? Also, i have to bloated feeling in my back when im NOT gassy or have to go to the bathroom. Its there other times as well.

Thanks for the follow up query..

I had noted that the abdominal ultrasound was 11 months old..

The cancer doesn't develop within months..It develops over years usually.

Also there are no other supporting symptoms of cancer like loss of appetite,weight loss..

Bloating in your case regardless of diet, might be related to a gynecological issue like polycystic ovarian disease which can develop in months and is common in patients of hypothyroidism too.I would advice you to get evaluated by a gynecologist to rule out PCOD with ultrasound ovaries and thorough clinical examination.Also you can get your doubts regarding kidney cancer solved.
