What Does My Ultrasound Scan Report Indicate?

Question: Good morning doctors, I am 4 wks and a few days pregnant and had my first ultrasound a few days ago to check for placement due to mild cramping I've been having (no bleeding). They confirmed the sac was in the uterus and not tubes however discovered there were 2 sacs. The doctor made a statement that he doesn't think one is a sac and instead is a "bleed". He didn't explain his rationale as to why and I was on such shock I dint ask.

My question to the doctors, does this look like a twin pregnancy to you? If so, can you tell from the sonogram whether it's identical or fraternal? Is it normal to have on and off mild cramping with no bleeding?

Most likely not a twin pregnancy.

Hello dear,
Sorry for a long delay in my response.
I have gone through your images and in my opinion your doctor opinion is right and it is most likely a single pregnancy with bleed not a twin pregnancy.
The reason behind this in your images,you are able to find out that one gestational sac has regular border and other is having irregular margins( which maybe due to bleed).
As you are cramping but not bleeding you should get your repeat ultrasound scan after two weeks. ( to see cardiac activities)
If any new symptoms arise,you should consult a Gynecologist for the same.
In twin pregnancy also it may happen sometimes that one will grow other will not.

I had my second ultrasound today and its confirmed to be a twin pregancy, 2 yolks seen..

Hello dear,
Congratulations!
If it is showing two yolk sac,it is confirmed twin pregnancy but we should wait and watch for heart beat to appear in both sac.
Sorry for my comment but it is only because your second sac is not having very regular ( smooth) margin.( which is very important for a healthy pregnancy) .
Have you done your beta hcg testing also?( it is more in twin pregnancy) .
